Outstanding Achievement Award

The Arkansas Public Health Association (APHA) Awards committee is accepting nominations for the Outstanding Achievement Award. Any member of APHA may nominate individuals for this award. The award recipient will be selected based on the strength of the nomination as judged by the criteria below. Awards will be presented during the annual APHA Conference in April 2020.

The deadline for nomination submissions has been extended to 5 p.m. on Tuesday, March 31, 2020.

The Outstanding Achievement Award was established in 1964 for achievement and success in one or more programs and/or specialties in public health. 

Nominations that do not address all questions and meet the criteria will not be considered. Answers do not have to be long but should be as specific as possible.

Tell us how the nominee has attained outstanding success in the program to which he/she was assigned and/or involved other programs and fields of public health.
Tell us how the nominee’s success required unusual effort and long hours of work and personal sacrifices.
Tell us how the nominee’s achievement is outstanding in comparison to other individual efforts in the same field of endeavor, related fields, or entirely different fields within the overall public health effort.
Tell us how the nominee’s achievement is verified and documented.
Please provide supporting documentation such as letters of support from colleagues, copies of reports, newspaper articles, tabulations, statistics, etc.). The more support you can provide, the better.
